Regular – 20 working days processing from personal appearance – P1100
Rush – 10 working days processing from personal appearance – P1350
For New Passports, here are the requirements:
NEW PASSPORT, single, 18 – 64 y/o
1. NSO copy of birth certificate (will not be returned; details must match with the details in the application form*)
2. 4 photos – passport size, wear professional top (with collar), royal blue background, remove contact lens, do not smile
3. application form (secure from us)
4. for personal appearance at DFA
5. 2 valid digitized IDs – preferrably government issued – SSS, voter’s ID, school ID
NEW PASSPORT, minor, 0-7 y/o
1. NSO copy of birth certificate (will not be returned; details must match with the details in the application form*)
2. 4 photos – passport size, wear professional top (with collar), royal blue background, remove contact lens, do not smile
3. application form (secure from us)
4. authenticated marriage contract of parents
5. valid passport of parent
NEW PASSPORT, minor, 8-17 y/o
1. NSO copy of birth certificate (will not be returned; details must match with the details in the application form*)
2. 4 photos – passport size, wear professional top (with collar), royal blue background, remove contact lens, do not smile
3. application form (secure from us)
4. for personal appearance at DFA, to be accompanied by parent
NEW PASSPORT, married woman
1. NSO copy of birth certificate (will not be returned; details must match with the details in the application form*)
2. 4 photos – passport size, wear professional top (with collar), royal blue background, remove contact lens, do not smile
3. 2 valid digitized IDs – preferrably government issued – SSS, voter’s ID, school ID
4. authenticated marriage certificate from NSO (will not be returned)
